Prospective coaches and assistant coaches must fill out the Volunteer Coach's Application and the Background Check Release.

Prospective Individuals should:

• Fill out both pages completely and sign both pages.
• Return forms to the Parks and Recreation office so you can get on the prospective coaches list.
